NHS Referral Information
Any patient already diagnosed and suitable for fertility treatment should be reviewed against the CCG criteria relevant to them, based on their GP location, to assess their eligibility for NHS-funded treatment. Patients eligible for NHS IVF should be provided with a list of all contracted providers (contained within the Consultant Referral to Specialist Provider for IVF Treatment form).
Any patients who require fertility assessment and diagnosis can be referred to the Outpatient Clinic at Addenbrooke's via the 'Suspected Infertility' proforma below. Patients will be seen by a team of specialists who will assess, diagnose and recommend what further treatment is required. This may include surgery, clomiphene citrate, ovulation induction or other fertility treatments such as IUI and IVF.
Patients not eligible for NHS treatment should be provided with information regarding what self-funded options are available to them. A Cambridge IVF patient information leaflet and brochure is available for download and printing at the bottom of this page.

Self-funding Referral Information
Patients who are concerned about their fertility and would like to have a Fertility Health Check or their initial investigations on a self-funded basis can be referred to Cambridge IVF along with those patients who are not eligible for NHS treatment.
Cambridge IVF is an NHS purpose-built assisted conception unit and is part of Cambridge University Hospitals. Income generated from Cambridge IVF is re-invested into the Women's and Children's Services Division at CUH. Based in Trumpington, it offers high quality, personalised care based on the very latest research.
Cambridge IVF is modern, bright and welcoming, and the building has been equipped with the latest technology and laboratory facilities. Set up in 2011, Cambridge IVF builds on 20 years as a fertility treatment centre at Addenbrooke's Hospital, and experience of running a successful satellite IVF service in partnership with other units.
